BlueRide Transportation service

BlueRideSM offers you a safe and reliable ride to covered medical, dental and pharmacy visits with providers in your plan’s network.

You can also use BlueRide to go to participating fitness locations, health and wellbeing classes, and Narcotics Anonymous and Alcoholics Anonymous meetings.1

Mobile Clinics

Our mobile clinic partnerships bring healthcare to your home in select counties. Services include well-child visits, immunizations for all ages, mammograms, bone density screenings, maternal health and more.

Nurse line

Registered and experienced nurses are available 24 hours a day, 7 days a week when you need medical advice or guidance on where to go for care.

Our nurses can also help you during or after your pregnancy.

Care Manager (Case Management)

If you have a major illness, injury or chronic health condition, you may be assigned a Care Manager. They will help you:

  • Better understand and follow your treatment plan, condition or disease
  • Find in-network providers and behavioral health support
  • Set up doctor visits and follow-up appointments
  • Answer questions between doctor visits

A Care Manager may be assigned to you based on your needs. Call Member Services with any questions.

Member Experience Advocate

A Member Experience Advocate can answer your health plan questions relating to plan benefits, coverage, claims, and finding an in-network provider (doctor).

Quitting tobacco program

Develop and maintain a quit smoking or vaping plan with support from a wellness coach.

Disease management

Case managers work with you by phone to create health goals, educate you about your condition and help you take control of your care.

Members will get a letter welcoming you to the disease management program, if you qualify.

County car seat and booster program

Infants and children under age 9 can receive a car seat and booster at no cost to you.* Children aged 9 or older who are unable to pass Minnesota’s Child Passenger Safety Law (5-point test) due to their size remain eligible for the car seat program.

*Restrictions apply. Pregnant Blue Plus members are eligible at 7 months gestation. Members are eligible for one car seat and one booster seat through this Blue Plus program. Eligible members living in Anoka, Cook, Dakota, Hennepin, Ramsey, Scott, St. Louis, and Blue Earth counties can contact Everyday Miracles directly. All others can contact their local County Public Health office.

Blue Plus Minnesota Senior Care Plus (MSC+)

Care Coordinator

A Care Coordinator is a licensed nurse, social worker or qualified healthcare provider who can help you:

  • Access home healthcare, community-based services and local resources
  • Understand your benefits and help get you enrolled in special programs
  • Find transportation to doctor appointments
  • Coordinate care between your primary care clinic and specialists
  • Manage your care before and after a hospitalization

Your Care Coordinator will typically reach out to you within the first month of joining the plan. If you have not been contacted by a Care Coordinator, call Member Services with any questions.

For Minnesota Senior Care Plus (MSC+) plan:

myFlexCard allowances

A myFlexCard Mastercard will be mailed to you for these benefits. You will be able to use the same card for over-the-counter (OTC) items and if eligible, healthy foods and household support.

Healthy foods and household support

If you have certain long-term health conditions, you may be eligible for an additional $275 every three months to pay for utilities, rent, and healthy food like fruits, vegetables, and whole grains.4

SilverSneakers® is an opportunity to improve your health, gain confidence and connect with your community. Stay active with fitness classes for ages 50+, on-demand workout videos and access to a broad network of fitness locations.

Music Therapy

Enhance your quality of life, improve physical and emotional health, and reduce stress with music therapy.

Members living in an assisted living or skilled nursing facility or foster home and experiencing certain mental health-related needs are eligible to receive up to 26 sessions per year with a board-certified music therapist.1

Comfort item

For members with a cognitive impairment diagnosis, a comfort item can help provide companionship and improve mood. Choose from a animatronic cat/dog/bird, or a baby doll.2

Disability hub

Disability Hub MN is a free statewide resource network that helps people with disabilities solve problems, navigate the system and plan for the future.

Friendly helper

Get up to 48 hours per year of in-person and virtual support services to increase community connections and help with everyday tasks such as light household chores, grocery shopping and more.

Medication Therapy Management

You can get free comprehensive medication reviews through the Medication Therapy Management (MTM) program as part of your plan. A specialized pharmacist will:

  • Talk about your list of medications and how you take them
  • Discuss ways to manage any side effects that bother you
  • Look for ways to lower your costs

After the review, you’ll get a written summary of what you talked about, and a recommended to-do list with specific suggestions.

Minnesota Aging Pathways

Minnesota Aging Pathways is an independent state program (not connected with any insurance company or health plan) that provides free help, information, and answers to your Medicare questions.

  • Comparing housing options
  • Connecting with help and services in the community
  • Moving out of a nursing home back into the community
  • And more
