What is a mobile clinic?

A mobile clinic brings health services to people in their homes.

This model of care helps improve access to health services in areas with gaps in access to care.

Mobile clinic options

The following mobile clinics serve Blue Plus members and vary in services, patient ages and service areas.

Odam Medical Group Mobile Clinic and Mobile Mammography

Services provided:

Children

  • Well-child visits
    • General health assessments
    • Weight, height and vital signs
    • Screenings, including vision, hearing and lead levels
  • Immunizations
  • Dental sealant application
  • Childhood asthma management

Adults and seniors

  • Mammograms
  • Bone density screening
  • Maternal health
    • Pregnancy testing
    • Prenatal and postpartum care
  • Pap smears and other routine screenings
  • Immunizations for adults and seniors
  • Fall risk assessments
  • Medication management
  • Other eligible treatments as identified during assessments and screenings

All ages

  • Diabetes testing and management, including foot care
  • Weight management
  • Testing and treatment of hypertension
  • Behavioral health evaluation and referrals

Service area: Becker, Benton, Cass, Clay, Crow Wing, Kanabec, Kandiyohi, Meeker, Mille Lacs, Morrison, Polk, Rice, Sherburne, Stearns, Todd, Wadena and Wright counties

Service days/times: Wednesday – Sunday by appointment

Hennepin Healthcare Mobile Pediatric and Postpartum Health

Services provided:

Children

  • Well child assessments and screenings, including growth development and lead screening
  • Fluoride treatments
  • Referrals to primary care provider or pediatrician
  • Child and teen vaccines and immunizations
  • Referrals to community resources

New mom care

  • Routine exam and care
  • Breastfeeding support
  • Birth control choices
  • Doula care
  • Help with:
    • Getting food
    • Mental health support
    • Treatment and recovery for alcohol and drug use
    • Community support
    • Scheduling future appointments

Infant care

  • First set of newborn immunizations
  • Newborn wellness exam
  • Infant safe sleep counseling
  • Car seat assistance

Service area: Hennepin, Ramsey, and parts of Anoka and Dakota counties

SecureBlue℠ (HMO SNP) is a health plan that contracts with both Medicare and the Minnesota Medical Assistance (Medicaid) program to provide benefits of both programs to enrollees. Enrollment in SecureBlue  (HMO SNP) depends on contract renewal.
