As a member of Blue Cross and Blue Shield of Minnesota and Blue Plus, you and your family can earn rewards for completing certain preventive activities.

Get started in three steps

Step 1: Print one voucher for each activity as needed
Voucher below must be emailed or postmarked within 90 days of the last required visit for each activity.

Step 2: Schedule a visit with your doctor
Take your printed voucher with you to your doctor visit.
Your doctor or clinic must sign the completed voucher during your visit and they will submit a claim for the visit.

Step 3: Receive your reward
Mail your completed voucher to the address on the back of the voucher. Just fold, tape and add a stamp.

You will receive your reward after your provider submits your claim and it's verified.1 Please allow at least 4 weeks2 for your provider to submit your claim and approximately 2-4 weeks to receive your reward through the mail.
Rewards cannot be processed without a verified claim from your doctor, and the voucher must be fully completed and legible.

Baby flu vaccine (age 0 to 2 years)
Vaccines are important at all ages.
Earn a $25 reward when your baby completes both doses of their flu vaccines by their second birthday.

Baby well-child visits (age 0 to 15 months)
Baby well‑child visits help your doctor track your child's health and growth. Babies should have six visits by 15 months of age.
Earn a $60 reward when your child completes all six baby well-child visits by the time they are 15 months old.

Well-child visits (age 16 to 30 months)
Well-child visits help your doctor track your toddler’s health and growth. Toddlers should have two visits before 30 months of age.
Earn a $25 reward when your child completes two well-child visits within 16-30 months of age.

Well-child visit (age 3 to 17 years)
Annual well‑child visits help your doctor track your child's health and growth. These visits cover age appropriate topics like nutrition, hearing and immunizations.
Earn a $25 reward when your child or teen completes one well-child visit per year between the ages of 3 and 17 years.
Preteen HPV vaccine series (age 9 to 12 years)
This vaccine series is recommended to protect against HPV infections.
Earn a $25 reward when your child completes the HPV vaccine series before they turn 13.

Diabetes care activity

Annual Diabetes A1C test (age 18-64 with diabetes)
It's important to monitor your blood sugar levels to manage your diabetes. The goal for A1C testing is to maintain an A1C level less than 7%.
If you have a diabetes diagnosis and are aged 18 to 64 years old, you can earn a $25 reward when you complete one annual A1C blood test.

Prenatal visit
It is important to start prenatal care as soon as you have a positive pregnancy test.
Earn a $75 reward when you complete a prenatal visit—either in person or through telehealth—during your first trimester (the first 3 months of pregnancy).
Note: Visits completed after the first trimester are not eligible and will be denied.
Please check with your provider for telehealth visit options.
Postpartum visit
Postpartum care after your baby is born can help you stay healthy.
Earn a $75 reward for completing one postpartum visit one to 12 weeks after you give birth.

Annual preventive care visit (age 18 to 64 years)
Annual preventive visits help ensure adults visit their doctor regularly to stay healthy.
Earn a $25 reward when you complete one annual adult preventive visit.
Annual mammogram (age 40 to 64 years)
An annual mammogram can detect breast cancer early, when it's easier to treat.
Earn a $50 reward when you complete one annual mammogram.
Annual chlamydia screening (age 16 to 24 years)
As a young adult, screening for sexually transmitted infections is an important part of care.
Earn a $25 reward for completing one chlamydia screening each year.
Annual cervical cancer screening (age 21 to 64 years)
An annual cervical cancer screening can detect cervical cancer early, when it's easier to treat.
Earn a $25 reward when you complete one annual cervical cancer screening.
Colonoscopy (age 45 to 64 years)
A colonoscopy can help detect colon cancer early, when it's easier to treat.
Earn a $50 reward for completing one colonoscopy screening. Reward may be claimed every 10 years.
Annual dental exam (age 2 to 64 years)
A regular dental checkup is important for healthy teeth and gums.
Earn a $25 reward for completing one dental exam per year.
1Incomplete or illegible vouchers will be denied.
2Providers are responsible for submitting claims within the timeline provided in their contracts. In some cases this can be anywhere from 90 to 180 days from time of service. Rewards will not be processed without a verified claim from your doctor.
This program may change or end at any time without notice. Limit one reward card per member per voucher each year. Please allow up to four weeks for your reward card to arrive in the mail.
Your rewards card cannot be used for gambling or to buy alcohol, tobacco, or firearm products. The Reward may result in a taxable event for either your or your plan sponsor. Consult your tax advisor.
