Dr. Aderinkomi: Thank you for your question, Robin. This is a common concern. In fact, many adults aged 65 and older manage two or more chronic conditions.
Seeing specialists is an important part of staying healthy. But it can also mean medications are prescribed by different doctors. Reviewing them together helps ensure everything continues to work safely and effectively as part of your overall care plan.
Your providers do work to coordinate care by sharing records and, in some cases, communicating directly. Still, taking time to periodically review your full medication list adds an extra layer of safety, particularly after hospital visits, new diagnoses or prescription changes.
For members with a prescription drug plan, one of the easiest ways to do this is by working with a pharmacist through our Medication Therapy Management (MTM) program. This no-cost service connects you one-on-one with a pharmacist who will:
• Review all prescriptions, over-the-counter medications and supplements
• Explain what each medication is for and how it works
• Check for potential interactions or duplications
• Confirm that doses and timing are appropriate
• Help identify ways to save on your medications
It’s a great time to ask questions. In some scenarios, a review may even identify ways to simplify your routine while keeping your conditions well managed.
There are also simple ways you can help keep your medications organized:
• Keep an updated medication list
• Bring it to every appointment
• Use one pharmacy when possible
• Take medications exactly as prescribed
