How it works

After you are discharged from an approved inpatient hospital or skilled nursing facility you will get a phone call from GA Foods to review the meal benefit and discuss delivery times.

Eligibility

As a member of Blue Cross and Blue Shield of Minnesota Medicare Advantage, you are eligible to receive two (2) meals per day for up to 14 days.

 

Meal delivery

Meals are delivered to your home at no cost, supporting your post-discharge nutritional needs by providing balanced meals to help you with a successful recovery.

Blue Cross and Blue Shield of Minnesota is working together with GA Foods to provide post discharge meals.

Members who have an approved medically necessary inpatient or skilled nursing facility stay will automatically be identified as eligible to receive post-discharge meals.

GA Foods then will begin outreaching to the member to confirm delivery address and obtain any dietary restrictions and/or meal preferences.

Your phone’s caller ID will say GA Foods. GA Foods will clearly state in their introduction that they are calling from GA Foods on behalf of Blue Cross. During the welcome call, you will discuss the first and ongoing deliveries, confirm your address, which transportation vendor will be used, review dietary needs and allergies, how to dispose of gel packs, and review how to store meals.

Delivery date and time is reviewed during your welcome call. The cooler is designed to be left safely on your doorstep throughout the day of delivery. You do not need to be present to accept the cooler or sign for it.

On average meals will be delivered within 24-72 business hours of the welcome call and will arrive via GA Foods Delivery or via a third-party delivery such as FedEx or UPS. The meals come fully cooked, frozen and pre-packaged in a box with packing and cooler inserts. There will be one week worth of meals. Each box contains an insert with meal menu, heating instructions and how to contact GA Foods if there are any issues upon arrival.

A unique GA Foods’ tracking ID is assigned to all deliveries. This tracking ID can be used on the front page of the GA Foods website: GAFoods.com. This ID can be obtained either in the client record on the customer portal or by contacting the Care Team 1-866-575-2772, TTY: 711, Monday through Friday 7 a.m. to 7 p.m. Central Time.
