When you join the Healthy Rewards program, you and your family can earn rewards for completing certain healthy activities.
How the rewards program works:

Enrolling is optional
When you complete your first activity, your rewards card will be mailed to your home. Each time you complete an activity, dollars are added to your reloadable rewards card.

Members 18 years and older can create their own Healthy Rewards account and receive their own card. For members younger than 18, rewards are added to the main household card.

Get Started with Healthy Rewards
Get started in three easy steps
Step 1: Create your account
Visit my.bravowell.com/bluecrossmn or call 1-833-786-8722, TTY 711.
You will need your member ID number located on the front of your member ID card, an email address and mailing address to get started.
Step 2: Schedule a visit with your doctor
Earn rewards when you or your family visit the doctor and complete any of the healthy activities that you’re eligible for in the program. Your doctor will let us know when you’ve completed the activity.
Step 3: Start earning rewards
Once your doctor lets us know you’ve completed your first visit, your reloadable rewards card will be mailed to you. Dollars are added to your card each time you complete a healthy activity.
Your rewards card can be used in most retail locations. It cannot be used for alcohol, tobacco, firearms or gambling.

Child and teen checkups
Baby flu vaccine (age 0 to 2 years)
Vaccines are important at all ages. A $25 reward will be added to your card when your baby completes both doses of their flu vaccines by their second birthday.
Preteen HPV vaccine series (age 9 to 12 years)
Preteen kids need their HPV vaccine series completed. A $25 reward will be added to your card when your child completes the HPV vaccine series by the time they turn 13.
Baby well-child visits (age 0 to 15 months)
Baby well-child visits allow your doctor to track the growth of your baby. Your baby should have six visits by age 15 months.
A $10 reward will be added to your card each time your child completes a baby well-child visit up to six visits ($60 or six visits maximum).
Well-child visits (age 16 to 30 months)
Well-child visits allow your doctor to track the health and growth of your toddler. Your toddler should have two visits before 30 months of age.
A $10 reward will be added to your card for the first visit and a $15 reward will be added to your card for the second visit.
Well-child visit (age 3 to 17 years)
Annual well-child visits are important to help ensure that children and teens are healthy. These visits cover many important age-appropriate topics like nutrition, hearing and immunizations.
A $25 reward will be added to your card when your child or teen completes one well-child visit between the ages of three and 17 years.
Diabetes care
Diabetes annual A1C test (age 18 to 64)
Taking this test is important to monitor your blood sugar level and measure how well your diabetes is managed. The goal for most adults age 18 to 64 with diabetes is an A1C level less than 7%.
A $25 reward will be added to your card when you complete your annual A1C blood test.
Maternal care
Prenatal visit
It is important to start prenatal care as soon as you have a positive pregnancy test. This visit helps you prepare for the care you need during pregnancy.
Earn a $75 reward for completing one prenatal visit either in person or via telehealth.

Postpartum visit
Keep yourself healthy after the delivery of your baby so you can keep your baby healthy.
Earn a $75 reward for completing your postpartum visit one to 12 weeks after you give birth.

Preventive care
Annual preventive care visit (age 18 to 64 years)
Annual check-ups help adults stay healthy by visiting a doctor regularly. These visits check physical and mental health, hearing, and vaccinations.
You can earn a $25 reward for completing one check-up each year.
Annual dental exam (age 2 to 64 years)
A regular dental checkup is important for healthy teeth and gums.
A $25 reward will be added to your card for completing an annual dental exam.
Annual mammogram (age 40 to 64 years)
An annual mammogram can detect breast cancer early, when it’s easier to treat.
A $50 reward will be added to your card when you complete a mammogram.
Colonoscopy (age 45 to 64 years)
A colonoscopy can help detect colon cancer early, when it’s easier to treat.
A $50 reward will be added to your card for completing a colonoscopy. Reward may only be claimed every 10 years.
Annual chlamydia screening (age 16 to 24 years)
As a young adult, screening for sexually transmitted infections is an important part of care.
Earn a $25 reward for completing one chlamydia screening each year.

Annual cervical cancer screening (age 21 to 64 years)
An annual cervical cancer screening can detect cervical cancer early, when it's easier to treat.
A $25 reward will be added to your card when you complete an annual cervical cancer screening.

Incomplete vouchers will be denied.
This program may change or end at any time without notice. Limit one reward card per member per voucher each year. Please allow up to eight weeks for your reward card to arrive in the mail. The reward may result in a taxable event. Consult your tax advisor. Rewards cannot be used for alcohol, tobacco, firearms or gambling.
