SecureBlue (Minnesota Senior Health Options)

Plan overview
SecureBlueSM is a Minnesota Senior Health Options (MSHO) plan that combines your Medicare and Medical Assistance into one plan. You must be 65 or older and have both Medicare Part A and Part B to qualify for SecureBlue.
There is no monthly premium for this plan. However, you may pay a copay (a dollar amount) for certain health care services or when you get a prescription.
SecureBlue helps you pay for:
- Basic medical care — doctor visits, medicine, hospital stays and more
- Dental care — basic dental services, plus two dental crowns, root planing and scaling*
- Treatment for mental health issues and substance abuse
- Preventive care, such as flu shots
- Eye exams, glasses and glasses upgrades
- Hearing tests and hearing aids
- Emergency care

When you have SecureBlue, you choose a doctor or clinic from our network to be your primary care clinic. They will get to know you and keep all of your medical records in one place.

A pharmacy network is a list of pharmacies (drugstores) where you can get medicine at lower prices. Choose from thousands of pharmacies in our network. You may have to pay more for your medicine if you don’t use a pharmacy that’s in our network.

This SecureBlue health plan has a list of drugs that it covers. It’s called a formulary. You may pay more for drugs that are not listed on the formulary.

Virtual care
Get access to doctors online who can answer questions, provide a diagnosis for common health concerns and prescribe medication (when appropriate).

Dental services
SecureBlue covers basic dental services, plus two dental crowns, root planing and scaling and an electric toothbrush.
For dental care, use the Delta Dental online dental directory. You must select the "Minnesota Select Dental Network" to view dentists that are in-network.

Specialized support
Care Coordinator
A Care Coordinator is a licensed nurse, social worker or qualified healthcare provider who can help you:
- Access home healthcare, community-based services and local resources
- Understand your benefits and help get you enrolled in special programs
- Find transportation to doctor appointments
- Coordinate care between your primary care clinic and specialists
- Manage your care before and after a hospitalization
Your Care Coordinator will typically reach out to you within the first month of joining the plan. If you have not been contacted by a Care Coordinator, call Member Services at 1-888-740-6013, TTY 711 with any questions. The call is free.
Case Manager
If you have a major illness, injury or chronic health condition, you will be assigned a Case Manager. They will help you:
- Better understand and follow your treatment plan, condition or disease
- Find in-network providers and behavioral health support
- Set up doctor visits and follow-up appointments
- Answer questions between doctor visits

Medication Therapy Management
If you are taking multiple medications, it is good to have a routine check-up of them. You can get free medication reviews through the Medication Therapy program as part of your plan. A specialized pharmacist will:
- Talk about your list of medications and how you are doing
- Discuss ways to manage any side effects that bother you
- Look for ways to lower your costs
Afterward, you’ll get a written summary of what you talked about, and a recommended to-do list with specific suggestions.

BlueRide℠ Transportation services
BlueRide offers you a safe and reliable ride to covered medical, dental and pharmacy visits with providers in your plan’s network.
You can also use BlueRide to go to participating fitness locations, health and wellbeing classes, and Narcotics Anonymous and Alcoholics Anonymous meetings.**
Health and wellness
CVS over-the-counter allowance
Members have an allowance of $150 per quarter to purchase select generic, over-the-counter (OTC) health and wellness items from a CVS catalog.
Music therapy
Enhance your quality of life, improve physical and emotional health, and reduce stress with music therapy. Members living in an assisted living or skilled nursing facility or foster home and experiencing certain mental health-related needs are eligible to receive up to 26 sessions per year with a board-certified music therapist.**
Blue Plus Health Self-Assessment
Complete the online health assessment so we can connect you to programs that can help manage your care. You will receive a personal report that describes the impact of each risk factor on your health.
Friendly helper
Get up to 60 hours per year of in-person and virtual support services to increase community connections and help with everyday tasks like light household chores, grocery shopping and more.

Animatronic pets
For members with dementia or Alzheimer’s disease, animatronic pets can help provide comfort and companionship and improve mood. Choose from a cat, dog or bird.**

Household support
Get help paying your utility bills and rent with a monthly allowance of $120. This is available for members experiencing certain chronic conditions. Utility company and landlord must meet certain qualifications to be eligible.**

Appointing a representative

Rights and responsibilities upon disenrollment
When you leave or end your membership in SecureBlue (HMO SNP) it's called a disenrollment. A voluntary disenrollment is when you decide to leave the plan. You can leave the plan at any time. Leaving our plan does not impact your eligibility for Medicare and Medicaid (Medical Assistance). An involuntary disenrollment is when the plan must end your membership. There are limited situations when you may not choose to leave, but we are required to end your membership. An example of this is if you provided fraudulent information on your enrollment form or allow abuse of your enrollment card.
If you are disenrolled for any reason, the plan will notify you of the date your membership will end and the reason for it ending. If you have questions about disenrollment, please contact Members Services at 1-888-740-6013, TTY 711.
