Plan overview

Blue Advantage Families and ChildrenSM from Blue Plus is a Minnesota Medical Assistance program (Minnesota’s state Medicaid program). This Minnesota Medical Assistance health care program is for families, children under age 21, pregnant women and adults under age 65 who receive Medical Assistance.

There is no monthly premium for this plan.

Blue Advantage Families and Children helps you pay for:

  • Basic medical care — doctor visits, medicine, hospital stays and more
  • Dental care — check-ups, cleanings and fillings*
  • Treatment for mental health issues and substance abuse
  • Preventive care, such as flu shots and screenings
  • Eye exams and glasses
  • Emergency care

How to enroll

To enroll in the Blue Advantage Families and Children plan, you must first qualify for medical assistance from the Minnesota Department of Human Services. The application process depends on your age, if you have children and whether you have a disability.

Blue Advantage Families & Children is available in all Minnesota counties except for Ramsey, Scott and Itasca counties.

Enrolling for renewing members

If you have a Blue Advantage Families and Children plan, the Department of Human Services (DHS) must redetermine your eligibility once a year to see whether you are still eligible for health care coverage. This process is called a renewal.

Find a pharmacy in network

A pharmacy network is a list of pharmacies (drugstores) where you can get medicine at lower prices. Choose from many pharmacies in our network.

You may have to pay more for your medicine if you don’t use a pharmacy in our network.

Prescription drugs

This Blue Plus health plan has a list of drugs that it covers. It’s called a formulary. You may pay more for drugs that are not listed on the formulary.

Dental services

Blue Advantage Families and Children covers basic dental services, including check-ups, cleanings and fillings.

For dental care, use the Delta Dental online dental directory. You must select the "Minnesota Select Dental Network" to view dentists that are in-network.

Case Manager

If you have a major illness, injury or chronic health condition, you will be assigned a Case Manager. They will help you:

  • Better understand and follow your treatment plan, condition or disease
  • Find in-network providers and behavioral health support
  • Set up doctor visits and follow-up appointments
  • Answer questions between doctor visits

A Case Manager will be assigned to you based on your needs. Call Member Services at 1-800-711-9862, TTY 711 with any questions. The call is free.

Disease management

Case managers work with you by phone to create health goals, educate you about your condition and help you take control of your care.

Members will get a letter welcoming you to the disease management program, if you qualify.

BlueRideSM offers you a safe and reliable ride to covered medical, dental and pharmacy visits with providers in your plan’s network.

Receive a free smartphone and 3GB data, 350 minutes and unlimited text messages each month through SafeLink.

Blue Plus members receive 200 additional minutes upon enrollment into the program and another 100 bonus minutes during their birth month each year.
