 ### Initial Enrollee Screening purpose and benefits

The screening helps identify immediate or ongoing healthcare needs you may have and to help connect you with the healthcare and community resources available to you through your benefits.

Blue Plus will also identify any dental, physical/mental and health/social needs you may have based on your responses and have a nurse or care team member connect you to programs that can help manage your care.

Answering the questions will not change your benefits. The screening helps us get to know you and what plan services may help you the most.

Information you share with us is Private Health Information and will not be shared unless it is used to provide additional support and resources.

#### **By answering a few questions you can:**

- Sign up for a care management program to help you manage your health
- Connect into pregnancy programs and rewards to help you have a healthy pregnancy
- Access free tools to quit smoking or tobacco use
- Tell us the language you use at home so we can mail your information in the language that you prefer
- Access free transportation to your medical appointments

**Answering the screening questions will not change or impact your health plan eligibility or benefits**

If you have any questions or concerns, please call Member Services at **1-800-711-9862**, TTY **711**, Monday to Friday, 8 a.m. to 5 p.m. Central Time. The call is free.

You can also call Member Services to request a copy of the screening.



 

 

 

 

 

Taking the screening is optional.

Initial Enrollee Screening is not available to SecureBlueSM and Minnesota Senior Care Plus members.
