## Access health plan information

Even if you are a family member, you’ll need written permission from your loved one to get access to their health plan information.

For you to access health plan information from our files, including their eligibility, payment and claims information, your loved one will need to complete, sign and send or submit the Authorization for Disclosure of Health information.

## Help manage your loved one’s care

A Power of Attorney is a legal document that gives you the power to act in your loved one’s place.

If your loved one ever becomes mentally incapacitated, you’ll need a Power of Attorney signed and filed with the state government to make decisions about your loved one’s medical care and finances.
