How do I find out if I qualify?

To find out if you are eligible for a Blue Plus plan, you must first fill out a Minnesota Health Care Program application. Free help is available to assist you with completing the application. Your application will be reviewed, as well as where you live, to determine if you are eligible for a Blue Plus plan.

Here are your options:

  • Call the Minnesota Health Care Programs help desk at (651) 431-2670 (in the metro area) or call toll free at 1-800-657-3739. TTY/TDD users 711, or through the Minnesota Relay direct access numbers at 1-800-627-3529 (TTY, voice, ASCII, hearing carryover), or 1-877-627-3848 (speech-to-speech).
