Outreach & resources

The Blue Plus Partner Relations team focuses solely on public programs and fosters relationships statewide to improve Minnesota’s communities. The team works with county public health agencies, human services agencies, care systems and their staffs to provide information, resources and education and to achieve better health outcomes for our members.

Screening tool helps identify programs for your clients

Many people qualify for Minnesota Health Care Programs and don’t know it. Sometimes people who know they qualify don’t enroll for the health care help they need because of a complicated application process, language problems or other barriers.

Bridge to Benefits, an initiative of Children’s Defense Fund Minnesota, aims to improve the well-being of families and individuals by linking them to public work support programs and tax credits. Bridge to Benefits uses a quick and simple online screening tool ExternalLink to help families find out if they may be eligible for public work support programs, including:

  • Health Care Programs (MinnesotaCare and Medical Assistance)
  • Child Care Assistance Program
  • Energy Assistance Program
  • School Meal Program
  • Food Support Program
  • Earned Income Tax Credit (EITC)
  • Working Family Credit
