How do I find out if I qualify?
To enroll in a Blue Plus plan, you must first fill out a Minnesota Health Care Program application. Free help is available to assist you with completing the application.
Here are your options:
- Call or visit your local county human services office
- Community organizations in your area
- Call the Minnesota Health Care Programs help desk at (651) 431-2670 (in the metro area) or call toll free at 1-800-657-3739. TTY/TDD users 7-1-1, or through the Minnesota Relay direct access numbers at 1-800-627-3529 (TTY, voice, ASCII, hearing carryover), or 1-877-627-3848 (speech-to-speech).

If you are 65 years old or over, call:
- Senior Linkage Line at 1-800-333-2433
- Medicare at 1-800-MEDICARE (1-800-633-4227).
TTY users should call 1-877-486-2048, 24 hours a day, 7 days a week - The Social Security Office at 1-800-772-1213, between 7 a.m. and 7 p.m., Monday through Friday; TTY users should call 1-800-325-0778.
If you are age 18 through 64 and have a disability, call:
