The Aware Gold® plan offers generous benefits, access to an extensive provider network and minimal cost sharing for your employees.

Benefit highlights

  • No deductible if your employees seek care in the network (a deductible is a set amount your employees pay each year for covered health care services, other than preventive care, before the health plan begins paying for the covered health care services)
  • Office visits for a $25 copay and in-network hospital services covered at 100 percent (a copay is a set amount that your employees pay for doctor’s office visits, prescription drugs, health care services or products at the time of service)
  • Retail health clinic visits covered at 100 percent (in network) with zero out-of-pocket costs for your employees
  • $45 copay to see a specialist
  • Preventive care covered at 100 percent in network with zero out-of-pocket costs for your employees. Preventive care includes well-child services, immunizations, prenatal care, routine physicals and eye exams
  • No maximum on the covered health care services the health plan will pay for your employees
  • Prescription drug coverage: $12 preferred generic drugs/$45 preferred brand drugs/$90 non-preferred drugs; mail-order savings available. See the covered drug list (please select the GenRx drug list).
  • Employees see the doctors and specialists they choose — no referrals needed
  • Access to the Aware® network, which offers your employees our broadest coverage and includes every hospital and 98 percent of doctors in the state
  • National coverage when your employees are traveling through the BlueCard® network
  • International coverage when your employees are traveling through the BlueCard Worldwide® network
