Pay-for-performance program for clinics 

About the program

Our pay-for-performance program, Recognizing Excellence, was developed to improve patient health and the affordability of care. The program is designed to reward clinics in two ways; Clinics who meet or exceed the established performance target for each measure and clinics whose performance improves from the previous year. Program measures include a number of clinical and preventive care measures, as well as the adoption of IT systems. Participation is voluntary. Measurements are evaluated and refined each year.

Measurements reports due: February 22, 2012

How to enroll

Please refer to the provider contract you have with Blue Cross and Blue Shield of Minnesota for specific terms of the program. Then, e-mail us your clinic’s interest to participate by October 1st. We’ll send you a confirmation and ask you to provide contact information. (We use your contact information to send you news and updates.)

How to gather and submit data

See the instructions provided below on how to complete the measures using audit forms. Once enrolled, if you choose to use our electronic audit tools we will be happy to send you a CD which you can download to your computer. To reduce administrative waste we require you to request a CD for the 2011 measurement. CD’s will be mailed in mid November 2011 for those who have requested one.
