Submitting your data
Please download the audit forms, technical specs and instructions (below) that apply to your clinic. NOTE: Universal Measures apply to the entire clinic (not each specialty). Only one set of Universal Measures should be submitted per clinic per program year.

Be sure to check the monthly newsletter we send you for updates to measurements and other information. Newsletters can also be downloaded in news & training.
Audit tool
Participants are mailed a CD with an audit tool program that can be downloaded to your computer. This audit tool helps make the submission process easier. The CD’s should be mailed November 2011. (If you are enrolled and did not receive the CD by mid November, please e-mail us.) To install the CD please use the Installation Guide. For help using the CD audit tool please reference the User Guide.
