The health coverage
See a doctor in Tier 1 of this health plan’s network and your employees pay:

  • $0 for three visits to an office or urgent care clinic
  • $0 for preventive care
  • $0 to visit a retail health clinic
  • You pick the level of employee cost-sharing with deductibles from $500 to $3,000. The higher the employee cost-sharing, the lower your monthly rate.

Get a prescription drug from our preferred drug list ExternalLink and your employees pay:

  • $12 for generic drugs
  • $45 for brand-name drugs

Prescription drugs not on the preferred drug list cost $90.

In Minnesota

This health plan comes with the Blue Performance RegionalSM network. It’s a great choice if you’re concerned with cost but don’t want to sacrifice choice. The Blue Performance Regional network includes 98 percent of doctors and 100 percent of hospitals in Minnesota but separates the health care providers into tiers, or levels, based on cost:

  • Tier 1 health care providers cost less
  • Tier 2 health care providers cost more

With Blue Performance Regional, your employees pay for higher-cost providers only when they choose to use them. They are rewarded with their best benefits and lowest out-of-pocket costs when they choose providers that cost less.
