Summaries of benefits and coverage (SBCs) provide standard details about Blue Cross and Blue Shield of Minnesota health plans. These PDF documents are for health plans for groups of 2-50 employees.

Note: These SBCS are for plans with an effectiive date of 7/1/12 or later unless noted otherwise. Options Blue HSA plans 476 & 576 and Community Blue plan 113 are the only plans with revisions for plans with an effective date of 7/1/13 or later.
