Networks for single and family plans

In Minnesota

You get choice and control in how you spend your health care dollars by selecting your network of doctors and hospitals in Minnesota.

1. Choose the Aware® network for the broadest access to doctors and hospitals at the lowest out-of-pocket costs:

  • It covers 98 percent of health care providers and 100 percent of hospitals in Minnesota
  • It’s a more traditional network
  • Your health plan coverage is the same no matter which in-network provider you choose

2. Choose the Blue Performance(SM) Regional or Blue Performance Enhanced network if you’re concerned with cost but don’t want to sacrifice choice:

  • It covers the exact same health care providers as the Aware network.
  • It’s a high-performance network: Health care providers are separated into tiers based on cost – Tier 1 providers cost less, Tier 2 providers cost more.
  • You pay different costs depending on whether you see a provider in Tier 1 or Tier 2. You will pay more out of pocket when you see providers in Tier 2.
  • You have to meet separate deductibles and out-of-pocket maximums at Tier 1 providers and Tier 2 providers.
  • You pay for higher-cost providers only when you choose to use them. You are rewarded with the lowest out-of-pocket costs when you choose providers in Tier 1.

Looking for the lowest cost network? You want Blue Performance Enhanced. It has Tier 1 health care providers available in select areas of the state. Blue Performance Regional is the next most affordable network. It has more doctors and hospitals in Tier 1 than Blue Performance Enhanced and has Tier 1 providers throughout the state.

Network options

National network

This health plan comes with the national BlueCard® network. It includes more than 92 percent of doctors and 96 percent of hospitals in the United States.

If you choose Aware® as your Minnesota network, your health plan comes with the BlueCard Traditional network. ExternalLink It has the broadest selection of health care providers nationally.

If you choose Blue Performance(SM) Enhanced or Regional as your Minnesota network, your health plan comes with the BlueCard PPO network. ExternalLink It has the greatest savings for services nationally.

International network

This health plan comes with the BlueCard Worldwide® network. ExternalLink It gives you access to doctors and hospitals in more than 200 countries.
