How to update your data

We recently made enhancements to our website to simplify the updating process. We added a link on the right side of the provider landing page, called Administrative updates . On this page, you’ll find common types of updates with links to the appropriate forms.

Change to submission forms

You can now complete commonly used forms electronically (in an interactive PDF) and e-mail them to us. If you do not have e-mail access, you can still complete the form electronically, print and fax it to us at the fax number listed on the form. Find forms in Forms and Publications .
